Breast reconstruction surgery is performed in order to restore a breast to normal appearance, shape and size. The medications are used for the comfort during the breast reconstruction surgery. General anesthesia is used for the surgery.
For repositioning a woman's own muscle, skin and fat in order to create or cover the breast mound, Flap techniques are used. In some cases, insufficient tissues are left on the chest wall for covering & supporting a breast implant after a Radiation therapy. Tissue expansion stretches the skin in order to provide coverage to a breast implant. Reconstruction of breast by using tissue expansion actually allows easy recovery than the flap methods, but it is a very lengthy process.
It requires several visits for the next six months post the expander placement for filling the device slowly through an internal valve for expanding the skin.
A second surgery is required to replace the expander if it is not designed to work permanently.
A breast implant surgery creates a breast mound. In fact, it can be an alternative or an alternative to the flap techniques. Silicon and saline implants are used for breast reconstruction surgery. Reshaping the Prominent Brow Bone - Improve Appearance

The need to change the shape of the forehead or brow bone is very uncommon. Reshaping the forehead or the brow bone (the bone below the eyebrows) is possible but there are different procedures that can be done based on the shape of the forehead and the brow bone.
The shape of the skull between a male and a female is often quite different. The male forehead often has fullness over the brow bone known as brow bossing or a supraorbital prominence with a flatter forehead above this area. The female forehead, conversely, has a more convex or curved forehead shape and little or no significant supraorbital bossing. Such forehead shapes confer a masculine or a feminine look.

The degree of brow bossing and the forehead shape helps determine what type of surgical recontouring needs to be done. With the exception of one other important consideration...the frontal sinus. The frontal sinus, an air-filled bone cavity, sits right under the brow bone and how developed it is will affect surgical choices. For this reason, any surgical efforts at forehead/brow modification should have a simple skull x-ray (side view) prior to surgery.
In those patients with mild to moderate brow bossing and thick skull bone over the frontal sinus (or are missing a frontal sinus), bone reduction by burring can be done with a nice result. When brow bossing is present but the bone thickness over the frontal sinus is thin, simple bone reduction contouring is impossible without entering the frontal sinus. Many try just a little bone reduction, without entering the sinus, but this does not make enough difference to justify the effort. Removal of only 1 or 2 mms of bone is not enough to make a difference. In these situation, one option is to open the frontal sinus, burr down the edges of the bone and put the 'outer lid' back in a more inward contour, thus preserving the frontal sinus. The other option is to obliterate and fill the sinus with a bone substitute material, making a more flatter brow contour with the bone paste or cement. (and not put the outer table of bone back) I have done both and both of them will work. If I can get a good brow contour and still leave the frontal sinus present and functioning, that is my preferred choice.
Any forehead and brow contouring requires an open approach through a scalp or hairline incision. The forehead skin must be 'peeled back' to get good access for the surgery. An endoscopic approach or more limited approach is not adequate to do a good job. In most females, the hairline and hair density patterns make an open approach possible. When this procedure is considered in males, the hair issue makes an open scalp approach potentially more problematic.
The most common patient, in my experience, for brow bone reduction is in female feminization surgery (FFS) where reducing the prominence of the brow bone helps in the overall facial conversion of the male to a female appearance. In a few select males with very prominent brow bones, this procedure can make a big difference in softening the more 'neanderthal' facial appearance.

The answer is in something most of us have plenty of: fat cells. Autologous fat transplantation - or body fat transfer - is done on an outpatient basis. Both the donor site (where the fat comes from) and the treatment site (where it's going) receive local anesthetic and the patient is usually sedated. The fat is taken out with a needle and syringe and "purified" to remove excess fluid, then re-injected at the treatment site. If you have recently had liposuction, breast reduction or a similar procedure, the fat extracted at that time can be frozen for later injection.

Body fat transfer has the advantage over collagen injections in that there is no possibility of a person developing an allergy to his own fat cells. It is also less expensive than most cosmetic procedures, such as the insertion of implants. They are not suitable for large areas, however, and will lend a full, rounded, but not a muscular appearance. Recovery from body fat transfer is quick, as there has been no traumatic invasion of the body. Bruising will fade within a couple of days, and swelling and redness should disappear in a few weeks. Activity may be limited for a while.
Disadvantages of Body Fat Transfer
Sites where there is little motion, such as the cheeks, stay firm longer than sites where the muscles move a great deal, such as the lips. Fat can also be transferred to the buttocks, arms, calves or other areas. Some experts believe that if it is used as breast augmentation material, it may interfere with mammogram results, while other surgeons routinely use it for this purpose.
There is a drawback, however. The body will reabsorb about 60 percent of the cells almost immediately. While the surgeon will purposely overfill to counteract this, the absorption may continue up to 99 percent. More injections are necessary for lasting results. If, after a series of injections, the recipient site reaches the point of saturation, then results can last a lifetime, but this may or may not happen. Three years is average. New techniques aimed at lengthening the results of body fat transfer are being used very day, and soon a permanent method may be found.

There is only one problem with all of these substances. They are all temporary. A synthetic chemical takes the form of tiny beads. When it is injected, the effects are longer lasting. Unfortunately, it is not widely available in the U.S.
Lip Augmentation for the Long Term
Grafting yields more extended results. The grafts can come from processed human donor skin. Again, this can come from the patient's own body fat (grafting is more effective than injection). There is also a fairly new material that is rolled up into a cylinder shape and placed into the lip through an incision.
The surgeon may also bring material from inside the mouth into the lips, but this is temporary, like injections. If you want to try out a new look before going doing a full surgery, start with on of the temporary procedures.
Recovery is much shorter for a lip augmentation procedure than for most cosmetic surgery. Injections require only a few days of rest, while it may be a week or two before you can return to normal activity after a graft.

Changing the Shape of the Face With Plastic Surgery

Making a face more square or angular can be done through bone augmentation, fat reduction, or both. I have found that there are two types of patients who come because they want a more defined facial look. First is the male patient (usually younger)who does not necessarily have a full or fat face but wants more definition at the defining points of the face. In some cases, I call this seeking the 'male model look'. The second type of patient is male or female who does have a fuller face and simply wants to be 'less round'. Their goals are not quite so precise as the first patient. Usually they are younger as well but can be middle-aged also.


The defining bony points of the face are the cheeks, chin, and jaw angles. Facial fat points are the cheeks and neck. Those searching for the 'male model look' are usually looking at the trio or combination of cheek, chin and jaw angle implants. The thinner the face is, the more profound the result will be with this triple implant approach. When you are placing three implants, or simultaneously changing three facial prominences, it is always best to not to overdo it. Large implants in all three places can create a very unnatural or artificial look. Cheek implants never look too good if they are too large. Subtle cheek changes are more natural.The proper size of a chin implant is easier to know because the amount of chin deficiency can be measured in profile. Most men should consider a more square chin implant style as that is often what this type of patient is trying to achieve. Jaw angle implants can rarely be too large as they are manufactured small anyway and the jaw muscles and thickness of tissue over the jaw angles can camouflage a great deal of the implant.
Those trying to deround a face must use a different approach. Fat removal in the neck (liposuction) and in the cheeks (buccal lipectomies) are an important element of derounding and is often done with a chin implant. In rare cases, cheek implants may be considered but they should be small as buccal fat removal will create the visual impression of some minor cheek augmentation as the area below the cheek moves inward. This type of facial derounding is more subtle than squaring a face with the most dramatic changes occurring in the neck and less in the cheek and side areas of the face.

Breast Enlargement Surgery

One of the most common operations performed in the UK cosmetic surgery industry is breast enlargement surgery, or breast augmentation. This involves the insertion of an implant into the chest wall to enhance the size of the breasts. The implants are inserted into a pocket either below the breast tissue or below the pectoral muscle, or in some cases partially under each. Implants may be round or teardrop shaped; their surface may be smooth or textured; the contents may be silicone or saline, and there are advantages and disadvantages with each option. Some implants are more expensive than others (probably because they’re better!)
Different surgeons will use different implants for various situations, and most will be more than happy to justify what they're using and why. This can sometimes be an important part of the discussion during the consultation and at the end of the consultation both your surgeon and you should be happy with the choices made.

If you’re thinking of having this procedure done, then you might want to have: the best implants, the best surgeon and the best aftercare. I have my own views about the best implants, your surgeon should be highly experienced and should be able to perform all types of breast cosmetic surgery and should be doing this regularly, and your aftercare should be for at least a year because if you do need a minor tweak to get things right then it will happen within a year.
POCKET PLACEMENT
The implants need to be placed within a pocket. There are three types of pocket and the best place to put the implant depends on the characteristics of the breast being enhanced, the implant being used and the desired look.
Submuscular Pocket
The submuscular pocket is often referred to as "under the muscle", as the pocket is made under the pectoral muscle of the chest wall. This pocket is often used in women who have minimal breast tissue so that the top of the implant is hidden under as much of your own tissues as possible to prevent a sharp take off of the breast and to reduce the chances of the implant being felt or seen. However, it can lead to the muscle being pushed forward on top of breast tissue which slides down as time goes by. This can lead to a “double bubble” problem where there are two bulges on the chest wall which may need further surgery for correction.
Subglandular Pocket
This pocket is often referred to as one which is made "on top of the muscle". The "sub" part of subglandular means "under" and the "glandular" part refers to the breast itself. So, the subglandular pocket is made under the breast, on top of the muscle. This pocket is often used when there is already a reasonable amount of breast tissue in someone who has little projection but is generally flat. It can also work well in someone who already has a reasonable breast volume but simply wants to be larger and is also used when someone wants to have an enhanced look following their surgery.
Dual Plane Pocket
The dual plane pocket is one that is made partially on top of the muscle and partially under the muscle. It involves splitting and releasing the muscle at exactly the right point so that you can have the benefits of having some of the implant above the muscle and some of the implant below it. This is often used in women who have a low take off and little tissue in the upper pole of the breast. By using a dual plane approach, what we achieve is to get the upper part of the implant hidden by the muscle but we prevent the appearance of two bulges in the breast (the double bubble effect) because of the low take off of the breast tissue by placing the lower part of the implant above the muscle.
IMPLANT TYPES
Breast implants come in three different varieties: the shape, the surface material texture and the filler. All implants have a silicone outer coating and implants of each type come in various different sizes.
Implant Shapes
Implants may be round or teardrop (anatomical) shaped. Both types have their advantages and disadvantages and the shape of implant can be discussed at the time of the consultation.
Implant Textures
Implants can be smooth or textured. Textured implants have a rough surface and smooth implants have a smooth texture. Textured implants have a lower risk of developing capsular contracture, which is hardening of the breast as the years go by.
Implant Fillers
Implants are commonly called silicone implants or saline implants and this refers to the fluid used to fill the implant. All implants have a silicone outer coating, but the filler material can differ. The most common filler materials we use nowadays are silicone or saline. Saline implants are filled with salt water at the time of the operation and a valve within the implant reduces the risk of saline leaking out of the implant causing deflation. Silicone implants have been extensively tested for safety. Different types of silicone are also available - some implants have thicker silicone within them, and the silicone will not leak out of the implant even if the implant ruptures. Other implants use softer silicone gel. These implants are more affordable and feel softer but can potentially leak silicone into your breast.
SIZES
Choosing the right implant size is a matter of balancing what you already have with what you want and what we can use. You might be measured you during your consultation and these measurements will determine the size of the implant that should be used, the pocket placement (ie below the muscle or over the muscle) the shape of the implant and the incision placement. The amount of nipple droop (or breast ptosis) will also tell whether you need to think about any other procedures to reshape the breast at the time of implant placement or at some other time in the future.

More and more rhinoplasty patients from the 80’s and early 90’s have requested revision rhinoplasty to meet today’s trend for a prominent nose profile and defining nose tip. Alternatively, rhinoplasty patients of the past may wish to otherwise improve the aesthetic appearance of the nose or increase functionality that has been compromised years after surgery.
Functional complications that have arisen from rhinoplasty procedures of the 80’s to the early 90’s include: ill-defined cartilage, unbalanced sidewalls of the nose, reduction in nose tip protrusion, and breathing impediments.
Today’s Options


There are often several reasons for a previous rhinoplasty patient to request a second revision procedure. This makes revision rhinoplasty surgery more complex than the initial surgery. By the same token, the advantage of secondary rhinoplasty today is that there are improved techniques available for patients to benefit from, including:
Grafting Techniques: There are several grafting techniques to improve functional and aesthetic concerns. Cartilage from the ears or ribs may be grafted to recreate a supportive structure for the bone and cartilage of the nose. The procedure requires up to 3 times the amount of time required for an initial rhinoplasty. In addition, recovery is much longer. Yet, the results can be quite complimentary from both an aesthetic and functional point of view.
Injectibles: Aesthetic issues can be improved through artistic techniques utilized through injectibles. Hyaluronics (Restylane and Hylaform) as well as calcium hydroxyapatite (Radiesse or Radiance) help to create a smooth profile and can improve depressed cartilage areas on the nose. Botox may be used to curve the lip upward, bringing balance to the features of the nose and face. Injectibles can be appealing because of the lack of downtime, immediate results that last 6 months to a year, and nominal fees when compared to grafting techniques.
Both grafting and injectible approaches are highly technique driven. They require the skill of a plastic surgeon with great expertise. It is important to select a plastic surgeon that has extensive experience in the procedure of choice for secondary rhinoplasty procedures.
Because of the special nature of the blood supply to the nose and surrounding area, it is possible for retrograde infections from the nasal area to spread to the brain. To reduce the risks follow the doctor's advice both before and after the surgery.

Surgical procedure:
Surgery can be performed under general anesthesia with local anesthesia depending on patient or doctor's preference. Incisions are made inside the nostrils sometimes; tiny incisions are also made on the columella, the bit of skin that separates the nostrils. The surgeon first separates soft tissues of the nose from the underlying structures, then reshapes the cartilage and bone which causes the deformity.
In some cases the surgeon may shape a small piece of the patients own cartilage or bone to strengthen or increase the structure of the nose. This is done for cosmetic reasons or to improve breathing and function of the nose. In rarer cases a synthetic implant may be used to reconstruct the nose if the normal structure of the bone and cartilage is badly damaged or weakened. Alloplastic synthetic are often associated with long term complications alternatively, cartilage from the septum, ear on rib may be used.
Recovery:
A tape dressing will cover the nose for one week, there may be some discoloration and swelling around the eyes which will improve over 5-7 days. One week is usually enough time for returning to work and social activities
